Current dashboard use the following exporter: https://github.com/spreaker/prometheus-pgbouncer-exporter

For correct work, you should add relabel_configs like this:

relabel_configs:
      - source_labels: [ __address__ ]
        target_label: hostname
        regex: 21.21.81.21:9127
        replacement: pg-slave2
